- Michigan Sales and Use Tax Certificate of Exemption (87K PDF)
- Uniform Sales and Use Tax Certificate (112K PDF)
- Return the Completed Application Forms
Once you’ve downloaded the new customer application forms, complete each form that applies to your company and return them to Turnkey Network Solutions. Forms can be returned via email to orders@tkns.net, fax at (616) 455-9845 or mail to our Executive Office:
